Thorizel is a drug that inhibits the enzyme protein kinase, thereby reducing the size and growth of atypical cells.
Produced in vials with concentrate and a special solvent: the vial with the active substance has a volume of 4 ml, the bottle of the solvent - 3 ml. One carton pack contains one vial of substance and one with a solvent.
Composition of
Thorizel contains 25 mg of tessirolimus, L-tocopherol, citric acid, propylene glycol and ethanol are used as auxiliary substances.
Ethanol, macrogol and polysorbate are present in the solvent. The suspension of the active substance in the vial is represented by a yellowish solution, and the solvent is a clear liquid.

Indications for use
Torysel is indicated for the development of renal cell carcinoma of a widespread nature.
Contraindications
It is strictly forbidden to take the drug to pregnant women and women breastfeeding, if there is an increased sensitivity to the components of the drug.
It is not advisable to use Torizel for the treatment of patients with severe or severe renal or hepatic insufficiency.

Mechanism of action
When a person enters the human body, tessirolimus binds to proteins specific for each cell, which in turn inhibit the activity of the mTOR kinase.

Instructions for use of Torizela
For the treatment of renal cell carcinoma, it is necessary to administer a Thorizel preparation of 25 mg by intravenous drip infusions once a week.
The course of therapy is continued usually until the appearance of the first toxic signs in the body or until the desired effect and suppression of tumor growth are achieved.
Dosage does not depend on the degree of progression of hepatic or renal failure.
Along with Torizel, an antihistamine should always be prescribed. It should be administered 30-40 minutes before the infusion of Toryzel.
When there are signs of overdose and intoxication of the human body with the drug temsirolimus, it is necessary to interrupt the course for a while. If the intoxication syndrome does not pass, reduce the dose of Toryzel to 20 mg per week.
In order to prepare a solution for drip infusion, all the contents of the solvent in the package must first be carefully inserted into the vial with the active substance, carefully turning the bottle until it dissolves completely.
Direct rays of sunlight can cause damage to the microelements of the drug, so manipulation should be done in a darkened room. The resulting solution can be stored no longer than 24 hours at room temperature of the ambient air.
Immediately before the drug is administered to the patient, the nurse should add this concentrate to the physiological solution( volume of 200 ml) and mix gently.
One infusion should last from 30 minutes to 1 hour, depending on the reaction of the patient's body to the drug. If the prepared solution is not used for 6 hours, then it becomes unfit for use.
Side effects of
The most common pathological reactions during the infusion of Torysel's drugs are attacks of nausea and vomiting, loss of appetite and vomiting immediately after ingestion, aversion to food, edema of the limbs and face, skin rash on the skin throughout the body, a decrease in the number of red blood cells in the blood(anemia).

Overdose
Symptoms that develop with an overdose of the drug Thorizel have not been studied, no recommendations have been given. With the introduction of sufficiently large doses of the drug in one session, no adverse clinical syndromes occurred.
Special instructions
Special care should be taken to prescribe the drug Thorizel to people suffering from diabetes should be carefully monitored during the therapy for the patient's blood sugar level( it can significantly increase).
The drug increases the cholesterol content in the blood, therefore lipid-lowering medications are prescribed in parallel with Torizel.

Compatibility
The drug Torysel is not recommended to be diluted immediately with water for injection( only with a special solution), since the crystallization of the substance and the precipitation can occur.
Only physiological saline solution should be used as a solution, no others.
Preparations Rifampicin, carbamazepine, phenobarbital, which have a stimulating effect on the exchange of cytochrome enzymes, are not recommended to be used concurrently with Torizel, since it is possible to decrease the maximum concentration of tessirolimus in the blood.
Also, the use of temsirolimus along with inhibitors of cytochrome enzymes should be treated with caution, as they, like stimulants, change the concentration in the blood of this drug.
