A lot of medical theories have been put forward on the etiology of duodenal ulcer. However, in the long history of the disease, none of them has become dominant. Modern medicine about the duodenal ulcer says that the disease occurs because of three categories of factors:
- Endogenous;
- Exogenous;
- is hereditary-constitutional.
If you look at patients with a duodenal ulcer, then in many cases it is possible to detect previous other pathologies of the gastrointestinal tract, endocrine system, urinary tract or CVS.That is, you can trace the neural-reflex connection, when the diseases of the listed systems form pathological reflexes that disrupt the motor and secretory function of the duodenum and stomach.
A separate role is assigned to the pathogenic Helicobacter pylori bacterium, which occurs in 87% of patients. Although, given that there are more than 40 strains of this bacterium, and it is found quite often in healthy people, the theory that the bacterium is important rather than
- itself is gaining in popularity;
- is the immune response of the body;
- total number of bacteria on the mucosa;
- duration of infection;
- predisposition.
As for exogenous factors, an important role is played by the violation of diet and stress. At the same time in the adult population the main role in the occurrence of peptic ulcer is played by constant stresses and nervous strains, and in children the nutrition with a lack of protein, vitamins and microelements necessary for the mucous growth of the growing organism comes to the fore.
One of the most important factors in the etiology of duodenal ulcer is the hereditary predisposition. So it was possible to establish that most of the children who developed a duodenal ulcer, in addition to the fact that the disease was carried by one of the relatives( more often the father), occurs:
- I-blood group;
- lack of gastric mucus fucoglycoproteins;
- high blood levels of cholinesterase, acetylcholine or pepsinogen;
- antibody of the HLA system.
The pathogenesis of peptic ulcer of the duodenum
The clinic of the ulcer of duodenum is quite complex and diverse. Simpler to imagine the development of DKD disease can be in the form of a confrontation between the protective factors of the mucosa and aggressive factors in the development of YaB.If the balance of confrontation shifts towards aggressive factors, then the prerequisites for the development of the disease arise, and over time it arises first in the form of inflammation of the mucosa, and then as a full-blown disease.
