Pharmacological action of

Ifosfamide has antitumor, cytostatic effect, which is based on the alkylation of nucleophilic centers in polymer biomolecules. This leads to a violation of their synthesis.

Ifosfamide is a prodrug that is activated by microsomal enzymes of the liver through the hydroxylation of the carbon atom in the ring. As a result, an unstable active metabolite is formed. The substance can rapidly transform into stable inactive metabolites, which are excreted together with urine( 4-keto-phosphoside and β-carboxyophosphamide).

The drug is able to pass through the BBB in a small amount. In patients who have a history of impaired liver function, there may be a decrease in activity of Ifosfamide.

Cytostatic has a carcinogenic effect - in the experiments on rats, an increase in the incidence of leiomyosarcomas, breast tumors was recorded. Numerous tests confirm the mutagenic effect of Ifosfamide, so the drug is prohibited from use in pregnant women.

Instructions for use Ifosfamide: dosage

Cytostatic is administered intravenously( each infusion is continued for 30 minutes) for several days.

The course dosage should not exceed 300 mg / kg. The next course is allowed only 3-4 weeks after the normalization of the peripheral blood composition, elimination of side effects.

With a brief infusion over 30 minutes - 2 hours, the powder is dissolved first in water for injection to achieve a concentration of 40 mg Ifosfamide per 1 ml water( 0.2 g in 5 ml water, 0.5 g - 13 ml).The resulting solution is diluted with 500 ml of Ringer's solution.

The drug is administered for 6-7 days at 50-60 mg / kg per day. If the infusion is used for 5 days, the dose is increased to 80 mg / kg.

With continuous infusion over the course of the day, 125-250 mg / kg of Ifosfamide per day is used. After this, a break for 3-4 weeks is necessary.

Solution Ifosfamide is dissolved in 3 liters of isotonic sodium chloride solution or glucose. The solution should be used within 24 hours after preparation, stored in the refrigerator. Side effects of
