To achieve a good condition of the dermis, it is often necessary to eliminate various inflammations, skin diseases. Dermatologists in each case recommend certain drugs. One of the remedies that helps with atopic dermatitis is "Elidel".
Features of the preparation
"Elidel" are produced in Switzerland. The drug is usually recommended by dermatologists for the treatment of atopic dermatitis.
The popularity of medicines has gained because of its effectiveness. In the cream, hormones are not contained.
Dosage forms
The drug is available in the form of a cream that is intended for external use. It is sold in aluminum tubes of different volumes( 15 mg, 30 mg, 100 mg).
Composition of Elidel
The active ingredient of the drug is pimecrolimus. This substance is an ascomycin derivative.
Price
The cost of the drug depends on certain factors( the amount of medicine, the pharmacy rating, the city).The price can vary between 902 - 957 rubles for 15 mg. The cost of 30 mg is much higher - about 1556 rubles.
Pharmacological action
Pharmacological action of the drug is represented by the local anti-inflammatory effect.
Pharmacodynamics
Pimecrolimus, which is the main component of the drug, provokes selective inhibition of production, release of inflammatory mediators, cytokines from mast cells, T-lymphocyte cells. Thus, an anti-inflammatory effect is achieved.
After a specific pimecrolimus compound with the cytosolic receptor "Macrophilin-12", the main component of the drug inhibits calcium-dependent phosphatase "Calcineurin."When inhibiting calcineurin, inhibition of T-lymphocyte proliferation is noted, production, transcription of early cytokines in T-helper type 1, 2 is prevented. Early cytokines include y-interferon, IL-2, IL-10, tumor necrosis factor, IL-4, IL-5, granulocyte-macrophage colony-stimulating factor.
Suppression of a secondary immune response within isolated cell colonies of dermal T-helles that were taken from patients with atopic dermatitis occurs under the influence of pimecrolimus, tacrolimus.
Pimecrolimus, with its topical use, has a high anti-inflammatory effect. Such a strong effect of the main component is possible with systemic use of the drug. If the action of pimecrolimus in the treatment of atopic dermatitis is evaluated, it will be as high as that of high-activity corticosteroids( fluticasone, clobetasol-17-propionate).It does not provoke changes in the consistency of the dermis, its atrophy, suppresses the inflammation that arises in response to the action of skin irritants.
If you use the drug for children( 3 months - 17 years) for 6 weeks, itching, inflammation of the dermis( infiltration, erythema, lichenization, excoriation) will noticeably decrease. If you use a medication for about 12 months, there is a marked reduction in the incidence of sudden exacerbation of AKD due to pimecrolimus.
Pharmacokinetics
Pimecrolimus concentration in the blood is observed in very rare cases( atopic dermatitis affects 15 - 59% of the dermis area).In 77.5% the concentration of the main substance in the blood is very low( less than 0.5 ng / ml), in 99.8% in general a scanty( below 1 ng / ml).
When using a medicament to treat children with atopic dermatitis, the concentration of the active substance in the blood was fixed at a consistently low level. The level of drug concentration in the blood does not depend on the area of the dermis, duration of treatment. In 98% of cases the level of concentration is below 2 ng / ml, in 60% this level was below 0.5 ng / ml.
Indications
The medication is intended for topical use in the therapy of eczema( atopic dermatitis).The duration of the therapeutic course is usually established by the treating dermatologist.
The cream can be used for a short time( with the elimination of atopic eczema symptoms), for a long time( as a prophylaxis of exacerbation of the disease).It is also prescribed for various forms of eczema and urticaria( dermographic, cold), in the treatment of mixed seborrhea and granuloma of the skin.
Instruction for use
Treatment with the drug should begin when the first signs of the disease are detected. This is necessary to prevent a sharp exacerbation of the disease.
Adult
Apply a thin layer on the dermis affected by dermatitis. It must be rubbed until completely absorbed. Apply the cream to any area of the dermis( body, neck, face).The cream is used twice a day. Therapy should be discontinued only after the symptoms disappear completely.
- If symptom severity persists even after 6 weeks of treatment, a re-examination of the patient is necessary to confirm the diagnosis of atopic dermatitis.
- If the first signs of a recurrence of dermatitis are found, you need to resume treatment with "Elidel".Given the fact that the drug is endowed with insignificant systemic absorption, there are no definite restrictions in the daily dose.
For children
- Until two years of age, dermatologists use the drug is not recommended.
- The elder children are given a drug 2 times a day.
The duration of therapy is established by a dermatologist. The duration of therapy depends on the severity of the lesion of the dermis, its stage. Apply the preparation with a thin layer.
In pregnancy and lactation
There are no specific data regarding the use of the drug in pregnancy. Specialists did not find a direct, indirect effect of the cream on fetal development, childbirth, postnatal development of the baby.
The drug is excreted with milk, so you should use it carefully during lactation. Due to the minimum degree of absorption of the drug into the blood, local application is allowed. The potential risk to humans is negligible. It is forbidden to apply ointment on the area of the breasts to nursing women.
Contraindications
Among the contraindications, we point out:
- children's age( children under 3 months of age can not be used because of lack of data on safety, effectiveness of cream use by children);
- weakened immunity;
- severe forms of inflammation, lesions of the dermis;
- increased sensitivity to thymicroimus, other components of the drug;
- of Netherton syndrome( there are no specific data regarding the safety of use for such patients);
- acute viral, fungal, bacterial infection of the dermis.

Doctors do not recommend using a medication to apply to the vaccination area until the local manifestations of post-vaccination reaction disappear completely.
Reviews
About the cream more positive feedback. It is often used because of its efficiency, ease of use. The only drawback is its high cost.
